New Patient Appointment Details

Welcome to Garden State Rheumatology Consultants (GSRC)! 

To help you get started as a new patient here, we thought it would be helpful to outline what to expect before and during your first visit at GSRC.  So the goal of this page is to walk you through the process of becoming a new patient, including contact, scheduling, billing and the expectations we have for you as our patient.

 

How do I make an appointment?

    • Call 908-977-1677
    • Have your insurance card(s) ready

What do I need to do before my New Patient appointment?

Before your first appointment, to make it as efficient as possible, please try to complete these steps:

    • Verify your insurance is active and in-network with GSRC; if not, speak with our staff about your options (out-of-network or self pay).
    • Complete the required forms online: we will send you an email and/or a text message to your cell phone, with the link to our medical intake form, office policy, and HIPAA notice.  You can fill out the forms before your visit, and it is secure and HIPAA compliant!
    • Request records and recent blood test results from referring doctors if instructed.

What do I need to do on the day of my New Patient appointment?

    • Remember to bring your ID, your insurance card(s), a form of payment if you have copay, co-insurance, and/or deductibles, and your medication list or medication bottles.
    • We kindly ask all patients to wear a face mask during their visit.

How much will my visit cost?

It all depends on your insurance plan, copayment, co-insurance, and deductibles.

    • If we are in-network with your insurance plan (a.k.a. we take your insurance), as a courtesy, after you send us your insurance card(s), we will contact your insurance for your copay, co-insurance, deductibles, and provide you a cost estimate for your visit within 1-2 business days.  However, it may not be the final cost to you, because your insurance plan ultimately determines your responsibility for the medical care you received.
    • If we are out-of-network with your insurance plan (a.k.a. we do not take your insurance) and you choose to be seen as a self-pay patient, once your appointment is scheduled, we will provide your a fee schedule and cost estimate by SMS text message or email.
